I go to pick up loot, like new weapons or health pickups etc, but it just isn't in my inventory at all. Doesn't seem to effect unique gear like V's pistol or Royce's "Chaos" handgun, but regular gear like armour and weapons just does not seem to appear.

Fix: Your middle mouse button is most likely mapped to a dpi shift or another button. So just turn it back to default. Then your unequip option will work. I have a G pro and went on g hub to change my middle mouse back to default.
